* dtable.cc (dtable::build_fhandler): Remove debugging output which uses 'cb'.
* exec.cc (execvp): New function.
(execvpe): Ditto.
* fhandler.cc (fhandler_base::fhandler_base): Use constructor initialization.
* fhandler.h (fhandler_tty_common::fhandler_tty_common): Ditto.
