The default PSAPI_VERSION is controlled by WIN32_WINNT, which we set to
0x0a00 when building utils since 48a76190 (and is the default in w32api
>= 9.0.0)
In order for the built executables to run on Windows Vista, we must also
define PSAPI_VERSION as 1 (otherwise '#define GetModuleFileNameExA
K32GetModuleFileNameExA' causes a 'The procedure entry point
K32GetModuleFilenameExA could not be located in the dynamic link library
kernel32.dll' error at run time).
Also drop uneeded psapi.h from dlfcn.cc (31ddf45d), resource.cc
(34a6eeab) and ps.cc (1def2148).

This causes K32GetModuleFileNameEx to be used for GetModuleFileNameEx,
which isn't available on Windows Vista.
Define PSAPI_VERSION as 1 for the built executable to work on Windows
Vista.
If the process exited with e.g. STATUS_DLL_NOT_FOUND, also process the
file to look for not found DLLs.
(We currently only do this when a STATUS_DLL_NOT_FOUND exception occurs,
which I haven't managed to observe)
This still isn't 100% correct, as it only examines the specified file
for missing DLLs, not recursively on the DLLs it depends upon.
Borrow a trick from strace to lessen occurrences of "??? => ???" in ldd
output. Specifically, if the module name isn't found in the usual place
in the mapped image, use the file handle we have to look up the name.
Ensure that ldd always stops when the exception is flagged as
non-continuable.
Also arrange for ldd to exit with a non-zero exit code if something went
wrong which prevented us from listing all dynamic dependencies.

set_entry_point_break() uses GetModuleInformation to fetch the
address of the exe's entry point. However, just as with
lpStartAddress from the CREATE_PROCESS_DEBUG_EVENT event, the
returned address is only computed from the PE file header. It's
not actually the entry point in memory, if the executable is
relocated (ASLR). See

Convert this to using the info from CREATE_PROCESS_DEBUG_EVENT
combined with the offset from the PE file header's AddressOfEntryPoint
to deal with relocation.

So far ldd terminates the inferior process as soon as some thread
is started. Apparently threads are started from even ntdll.dll
before the main thread of the application is started. As a result
the dll list is cut short since ldd terminates prematurely.

Bump GPLv2+ to GPLv3+ for some files, clarify BSD 2-clause.
Everything else stays under GPLv3+.
New Linking Exception exempts resulting executables from LGPLv3 section 4.
Add CONTRIBUTORS file to keep track of licensing.
Remove 'Copyright Red Hat Inc' comments.

option handling to use the same style throughout all Cygwin utils.
Throughout use program_invocation_short_name to refer to current
process name in Cygwin executables.
* utils.sgml: Align documentation to above change. Add missing
sections for getconf, ldd, and setmetamode.
* strace.cc (proc_child): Avoid compiler warning.
(head): Move earlier.
(saw_file): Ditto.
(print_dlls): Reset head here. Record seen dll names so they will not
potentially be printed later.
(process_file): Remove clearing of head here.
